MUNDELEIN POLICE DEPARTMENT ARREST SUMMARY

The following subjects have been charged with a crime.  The charge is merely an accusation; the defendant is presumed innocent unless proven guilty in a court of law.


DATE: 09-27-16

TIME: 1:17 am                                   

REPORT NUMBER: 16-09174                       

LOCATION OF OCCURANCE: Lake Street/ Courtland Street

CHARGE(S): Failed to Yield Turning Left, Defective Windshield, DUI, No Insurance                            

SUSPECT INFORMATION: Hamm, Brian W, 48 year old male, 170 N. Prairie Ave, Mundelein, IL    

BOND1: $1,000 Recog +DL                                            

COURT INFORMATION2:               10-14-16, 9:00 am, Waukegan

SUMMARY: Vehicle attempted to make a left turn, turning in front of officer’s squad car causing officer to take evasive actions to avoid an accident.  Officer made contact with driver and detected an odor of an alcoholic beverage coming from his breath.  Driver failed field sobriety tests.  Subject was arrested for DUI and No Insurance.

LOCATION OF OCCURANCE: 26132 N. Maple, Mundelein

CHARGE(S):  Expired Registration, No Seat Belt, Cellphone Use While Driving, No Insurance, Illegal Possession of Ammunition With Out a FOID Card.

SUSPECT INFORMATION: Mabe, Terry J, 36 year old male, 26132 N. Maple Ave., Mundelein       

BOND1: $2,000 Recog    

COURT INFORMATION2:               11-04-16, 8:45 am, Waukegan   

SUMMARY: Officer observed subject driving while using his cellphone, not wearing a seat belt and the vehicle had an expired registration.  Located in the vehicle, inside the center console was 2 Winchester 12 gauge shot gun shells.  Subject was arrested for possession of ammunition without a valid FOID card and No Insurance.

TIME:  5:10 pm                                 

REPORT NUMBER: 16-09227

LOCATION OF OCCURANCE: Diamond Lake Rd./ Route 83

CHARGE(S):  Expired Registration, Possession of Cannabis, Possession of a Controlled Substance

SUSPECT INFORMATION: Vazquez, Giovanni, 21 year old male, 26171 N. Oak Ave., Mundelein, IL             

BOND1: BH                                         

COURT INFORMATION2:               09-29-16, Waukegan     

SUMMARY: Officer observed that the vehicle had an Expired Registration.  Officer made contact with driver and while speaking to him the officer detected the smell of cannabis coming from inside the vehicle.  Officer located on the front passenger floor board a plastic container containing approximately 2 grams of cannabis.  Once subject was outside of vehicle, officer observed subject grab an item from his front pants pocket and throw the item in to the rear bed of the vehicle (pickup truck).  Officer located the item, one plastic baggie containing approximately .5 grams of cocaine.  Subject was arrested for Possession of Cannabis and Possession of a Controlled Substance.

CHARGE(S):  Speeding, Cellphone Use While Driving, No Insurance, Revoked Driver’s License

SUSPECT INFORMATION: Campechano-Perez, Gonzalo, 43 year old male, 607 Shady Ln, Mundelein, IL  

BOND1: $2,000 Recog                    

COURT INFORMATION2:               10-20-16, Mundelein    

SUMMARY: Officer observed vehicle speeding (46mph in a 35mph zone) and the driver was on his cellphone.  Subject was arrested for No Insurance and Driving on a Revoked Driver’s License.

TIME:  9:18 pm                                 

REPORT NUMBER: 16-09318

LOCATION OF OCCURANCE: Midlothian Rd./ Division St.

CHARGE(S):  Fail to Reduce Speed Causing an Accident, Improper Lane Usage, DUI, Illegal Transportation of Liquor

SUSPECT INFORMATION: Ledesma, Jaime, 31 year old male, 220 Bellevue Dr., IL               

BOND1: $3,000.00 Recog                              

COURT INFORMATION2:                 10-21-16, 9:00 am, Waukegan

SUMMARY: Officer responded to a report of a vehicle in a ditch.  Officers located subject outside vehicle (no injuries).  Officer detected odor of an alcoholic beverage coming from his breath.  Officer located on the floor board in front of front passenger seat were three 16oz cans of beer (Tighthead brand) with one of the cans open.  Subject was arrested for DUI and Illegal transportation of alcohol.

REPORT NUMBER: 16-09321                       

LOCATION OF OCCURANCE:  264 N. Prairie, Mundelein

CHARGE(S):  Criminal Damage to Vehicle

SUSPECT INFORMATION: Lazaro, Ricardo A., 24 year old male, 809 Walnut St. #2A, Mundelein, IL

BOND1: NTA     

COURT INFORMATION2:               11-06-16, 9:00 am, Mundelein

SUMMARY:  Officer received a report of Lazaro (domestic situation) banging on a vehicle parked in the parking lot and breaking a rear tail light.  Lazaro had left the scene prior to officer’s arrival.  Lazaro turned himself in to the police and was arrested for Criminal Damage to Vehicle.

LOCATION OF OCCURANCE: Route 45/ Lyndale St.

CHARGE(S):  Speeding, Suspended Driver’s License

SUSPECT INFORMATION: Carroll, Andrew, 32 year old male, 289 Briargate Dr., Grayslake, IL        

BOND1: $1,500 Recog                                    

COURT INFORMATION2:               10-20-16, 3:00 pm, Mundelein  

SUMMARY: Officer observed vehicle speeding (45mph in a 30mph zone).  Subject was arrested for driving on a Suspended Driver’s License.
